The staff of HFM Women's Health and the Women and Children's Center invite you to personalize your birth experience. The following worksheet will help you define those preferences to all involved in your care. Please know that you and your baby’s health and well being always come first. With this in mind your healthcare provider may recommend substitutes to your labor and birth preferences.
