According to Cheryl Reinke, Director of HFM Rehab Services, this collaboration ensures a smooth rehabilitation process for patients of Holy Family Memorial. “Our goal is to improve the continuity of care and quality for all of HFM’s patients admitted to the hospital and then discharged to Shady Lane.”
Reinke adds that this partnership aligns with HFM’s mission and commitment to the community. “We look for strategic partners such as Shady Lane who possess similar values, to collaboratively help improve the health of our community.”
