HFM earns 5-Star Rating from CMS

Holy Family Memorial has earned five stars—the highest ranking possible—from the Centers for Medicare and Medicaid Services (CMS) Overall Star Ratings for 2017. HFM in in the top nine percent of the 3,692 hospitals evaluated across the nation.

HFM Home Health Care named as a Top Agency of the 2017 ABILITY | HomeCare Elite

HFM Home Health Care has been recognized again as a Top Agency of the 2017 HomeCare Elite®, a recognition of the top-performing home health agencies in the United States.

American College of Cardiology NCDR ACTION Registry Platinum Award for 2017

For the third consecutive year, HFM has been recognized by the American College of Cardiology for its commitment and success in implementing a higher standard of care for heart attack patients. HFM is one of only 193 hospitals nationwide, and the only in Manitowoc County, to receive the American College of Cardiology’s Platinum Performance Achievement Award in 2017. Our leading-edge Cardiac Cath—the only 24/7 cardiac cath lab in Manitowoc and Sheboygan counties—allows our team to quickly and expertly diagnoses, treats and manages everything from chest pain, heart attacks and congestive heart failure, to coronary artery disease, myocardial infarction, arrhythmias and hypertension.

Mission: Lifeline® Silver Plus Receiving Quality Achievement Award, and Mission: Lifeline® NSTEMI Silver Quality Achievement Award from the American Heart Association

For top performance in following guideline-based heart care, which research shows can improve patient outcomes. HFM is the only hospital in Manitowoc and Sheboygan counties to receive this award in 2017. HFM’s average first medical contact to intervention is 45 minutes, just half the national benchmark of 90 minutes. This means our community and region can get award-winning heart care right here at home that only a handful of hospitals in the nation can even come close to matching!

Fifth time HFM was called out as being a frontrunner in the world of electronic health records of close to 3,000 hospitals reviewed. Being ahead of the curve in record security and innovative use of technology with e-visits and telemedicine means our community is in the top echelon of IT innovation.

HFM is the ONLY hospital in the county and one of only 4 from Wisconsin. This recognition puts care at HFM in the top 4% of the 2845 community hospitals in the US. Two years in a row!

High performing healthy workplace with a comprehensive program that achieves results. For a second year HFM leads the pack in Manitowoc County as a role model for a healthy workplace. In the last decade growth in our own health insurance premiums has been more than 60% less than the national average!

By placing HFM in the top 3% of national hospitals our performance and safety score means superb care for all. Two years in a row!

This prestigious award recognizes state organizations for work place excellence and those that have sharpened their competitive edge and are making an impact on the local economy. Due to the rigor of this Baldrige–based evaluation process, only an elite handful or organizations even attempt this. HFM’s commitment to continuous performance excellence and improvement in all aspects of care is validated by Mastery level performance. Only a few select hospitals pursue this rigorous path to excellence.

5-Star Home Health Patient Experience of Care rating from Centers for Medicare & Medicaid Services’ (CMS)

The star ratings are based on data collected for the Home Health Care Consumer Assessment of Healthcare Providers and Systems (HHCAHPS) Survey which evaluate patients’ experiences with home health agencies for quality of care, communication and overall satisfaction. Home can be the best place to get well and according to Medicare, HFM has the best caregivers to deliver that care.

Blue Distinction® Center for Maternity Care designation from Anthem Blue Cross and Blue Shield

This is a new national designation to help prospective parents find hospitals that deliver quality, affordable maternity care for new mothers and their children, along with evidence-based care and successful breastfeeding outcomes. HFM is proud to earn this designation and even prouder of the hundreds of new lives born safely at HFM each year.

Most Wired Hospital by Hospitals & Health Networks Magazine

One of only 8 hospitals of the 133 in Wisconsin to receive this award, HFM has long been a front-runner of using information technology successfully to assure secure medical information, critically accurate information and the newest technology in telemedicine and analytics. Being named to this top list means patients can feel safe in knowing their health care information is protected and that state of the art technology is used in their care.


UnitedHealthcare PATH Excellence in Patient Service Award

UnitedHealthcare has recognized Holy Family Memorial for helping improve outcomes for its Medicare Advantage members through review of care metrics and successfully addressing care opportunities. This significant award demonstrates HFM’s commitment to caring for all with excellence, compassion and respect. Proof of mission in action.

Community Outreach Award from HealthTru

HFM received this award because we have gone “above and beyond” in our community outreach efforts to improve access to care. HFM received the award for its onsite primary care clinic collaboration with several long-term senior living communities in Manitowoc which is based on HFM’s Right Care, Right Setting and Right Outcomes philosophy. As a result, residents have convenient access to a physician without ever having to step outside of their senior living community, improving care outcomes, reducing the need for ER visits and hospitalizations and greatly increasing resident satisfaction.

Mission Lifeline Gold Plus Receiving Award

The American Heart Association recognized HFM for achieving 85% or higher composite adherence to all Mission: Lifeline® STEMI Receiving Center Performance Achievement indicators for consecutive 24-month intervals, 75% or higher compliance on all Mission: Lifeline® STEMI Receiving Center quality measures, and First-Door-to-Device time of 120 minutes or less for transfers, to improve the quality of care for STEMI patients. A+ heart attack care right here in Manitowoc County, minutes from home, which is important because every minute matters with a heart attack.

Recognized for HFM’s commitment and success in implementing a higher standard of care for heart attack patients and signifies that HFM has reached an aggressive goal of treating these patients to standard levels of care as outlined by the American College of Cardiology/American Heart Association clinical guidelines and recommendations. Top 75 Cancer Centers in the United States

The HFM Cancer Center was named among the finest for cancer care in the entire country by the Commission on Cancer of the American College of Surgeons. It’s top notch care and outcomes are gold star! HFM is one of only five Cancer Centers in Wisconsin to receive this honor.

Trauma Center Level III Emergency Care

The Department of Health Services awarded our ER with this upgraded level of care certificate, based on our excellent ER services and meeting strict criteria for overall performance. When an emergency strikes, HFM ER is the best choice.
