Diabetes is a chronic, life-long, on-going condition. While you may see a physician or another health care provider several times a year, most days you are the one who controls your diabetes through monitoring, nutrition, exercise and managing your symptoms. Healthy Living with Diabetes is a researched and proven program designed to help you do that.
What is Healthy Living with Diabetes?
A 6-week workshop proven to help people with diabetes better manage their condition
This workshop is designed to help adults with type 2 diabetes or pre-diabetes learn self-management skills and increase their confidence in managing their diabetes. The workshop meets once a week for six weeks — 2-1/2 hours each session. Cost: $10.00 for entire six-week session.
Healthy Living with Diabetes complements existing treatments a participant receives.

The HFM Diabetes Center provides patients with the latest information on diabetes education, treatment and management. Through outpatient instruction, we can help you improve your diabetes and prevent or minimize diabetes-related complications. Our goal is to help you live an active, independent life.

To successfully manage diabetes, you have to be actively involved. DSME is a Medicare-covered service where patients can meet individually with a registered nurse and dietitian who specialize in diabetes management. You can get all your diabetes-related questions answered and attend four group classes, all to learn the vital information about healthy living with diabetes.
In addition to assisting those who have already been diagnosed with diabetes, the HFM Diabetes Center also offers a weight loss program that helps prevent diabetes. The Why Weight program provides education and resources to make lifestyle changes—such as nutrition and diet changes and increasing activity levels—to reduce the chances of developing diabetes.
