With the WHO On-Site Occupational Services you will facilitate the continuity of care in the rehabilitation process of your employees, whether it’s early intervention or follow-up care. Provide frequent communication of critical information to ensure a safe, cost effective return to work.

NEW - Disease Management
Disease Management will help employees manage and improve their wellness and potentially see their doctors less, there by IMPROVING THEIR HEALTH AND YOUR BOTTOM LINE.
Holy Family Memorial now offers a disease management program for employers. It is a journey of shared responsibility between employees and providers. HFM’s team works with adult family members who have coverage under their company’s own health care plan for diabetes, high blood pressure, and/or high cholesterol. This new system is grounded on:
- Evidence-based management
- Innovative teaching visits
- Team-based care of diabetes, high blood pressure, and/or high cholesterol.
Companies are encouraged to offer incentives to engage employees and spouses to participate – we have found this to be highly successful.
Disease management is a WIN/WIN journey of joint learning and health improvement – a personal and population-based proactive process that provide benefits to both employee and employer.

On-Site Nursing
WHO's On-site Nursing Model has been recognized and used by other healthcare providers.
- Administers first aid to injured/ill employees on-site and assesses and evaluates work related claims as well as management of personal injuries and illness.
- Under the supervision of Work Health Options Medical Director, administers conservative treatment to reduce absenteeism, and arrives at a resolution ideally without medical expenses.
- Makes it possible to monitor the effectiveness of the treatment plan used on-site or by various physicians.
- Provides employers and their employees with information to select physicians who can best meet their specific health care needs.
- Accelerates the rate of recovery for injured workers so that they may return to work with fewer lost workdays.
- Establishes a line of communication between injured worker, employer, physician and insurer. Acts as a neutral party, assessing the injured worker’s status and verifies that a specific diagnosis and treatment plan has been established.
- Assists the employer with recommendations for job modification and light duty if appropriate; assists in job analysis and verifies that the injured worker is working within restrictions and is able to perform job duties without difficulty.
- Promotes and actively participates in Wellness Initiatives. Provides disease management for high-risk employees.
- Participates in employer OSHA training and Safety and Wellness committees.
- Has an excellent understanding of Worker’s Compensation, OSHA Regulations, ADA (American Disability Act) laws and how this affects the injured worker and the employer.
In addition there is immediate in-house accessibility through the WHO Clinic, or Walk-In Care at our Harbor Town Campus. Holy Family Memorial’s Emergency Medical Services Department is also available at our Western Avenue Campus. Injury Care Van transportation, follow-up phone calls, and physician accessibility services are also offered.

Case Management & Consulting
Case Management representatives interact with you, your injured worker, insurance carriers, and physicians to speed your employee’s return to work, decrease lost time from accidents, decrease medical expenses and workers’ compensation costs, and quickly solve the claim in the best interest of your business and your employee. A Case Manager can also attend the doctor appointments. |
On-Site Clinic
Imagine you and your employees being able to see a doctor that is right next door. Reducing the number of visits employees make to off-site clinics, emergency rooms, and hospitals, will bring significant cost savings to employers by assisting in maintaining overall health and managing chronic health conditions.

The Business Connections Team will help you redesign benefit packages as necessary. An On-Site Clinic is a great way to provide a foundation for wellness and prevention initiatives in the workplace.
