Guided care planning is the process of planning for your future health care needs in the event you are unable to make your own health care decisions. Holy Family Memorial offers a free service where you can meet with a nurse navigator in your home, the clinic or the hospital. Together, you will discuss your beliefs, values and goals to create a plan that reflects your personal choices.

It’s tough to think about illness, but it’s good to write your wishes down. Plus, it will make it easier on your loved ones who may need to make decisions on your behalf.
