  • An International Certificate of Vaccination will detail the type of vaccination(s) you have received.
  • We’ll also help with any other documentation necessary for your destination.

We recommend scheduling your personal travel health appointment at least six weeks before you plan to travel to allow enough time for appropriate immunizations.

Travel Medical History Form

Once you have scheduled your appointment, print and complete the Travel Medical History Form and return to HFM Pharmacy at least 3 days before your scheduled appointment.
