Holy Family Memorial Primary Care and the Lakeshore Memory Clinic located at Felician Village, 1635 S. 21st Street in Manitowoc, are both reopening. HFM Primary Care was temporarily closed this past spring due to the COVID-19 pandemic. Lakeshore Memory Clinic is returning to its location at Felician Village after temporarily operating out of the HFM Reed Avenue Campus due to COVID restrictions.
HFM Primary Care at Felician Village will reopen on Monday, October 26. Dr. Jeffrey Allen is accepting patient appointments. The clinic will be open on Monday and Thursday from 7:30 a.m. to 5 p.m. and on Tuesday and Friday from 7 a.m. to 4:30 p.m. For more information or to schedule an appointment with Dr. Allen, call HFM Primary Care at (920) 320-2436.
Lakeshore Memory Clinic will reopen on Wednesday, November 11. The memory clinic will be open on the second, third, and fourth Wednesday of each month from 7:30 a.m. to 5 p.m. To schedule an appointment with Lakeshore Memory Clinic, call (920) 320-8600.
Patients are required to use the new exterior entrance for HFM Primary Care and Lakeshore Memory Clinic at Felician Village. The new entrance is located to the left of the main Felician Village entrance. Patients are required to wear a mask to their appointment and will have their temperatures checked upon arrival. Any patients showing signs of a fever or other respiratory symptoms should reschedule their appointment.
