Holy Family Memorial and Sharon S. Richardson Community Hospice Enter Preferred Provider Agreement for Hospice

Manitowoc, Wis., (July 25, 2022) – Holy Family Memorial, part of the Froedtert & the Medical College of Wisconsin health network, and Sharon S. Richardson Community Hospice (SSRCH) announced today that they have entered into a preferred provider agreement for hospice services. The preferred provider agreement goes into effect Sept. 1, 2022.

With the preferred provider agreement, Holy Family Memorial will discontinue providing hospice services on Sept. 1, 2022. Patients currently enrolled in Holy Family Memorial Hospice services will have the option to seamlessly transition their care to SSRCH or another hospice provider of their choice.
