HFM Chamber Clinic
The HFM Chamber Clinic is a convenient, one-stop medical and wellness benefits to help people proactively engage in health behaviors, which drive personal and professional success.
The HFM Chamber Clinic offers services at two convenient locations: Manitowoc and Sheboygan, beginning in 2019. Those who are The Chamber of Manitowoc County or Sheboygan County Chamber of Commerce members can go to either location.
Both locations offer primary, urgent, and preventative care. In addition to primary care, the Manitowoc location offers specialty services: physical therapy, wellness coaching, and emotional health counseling. There is a small annual investment of $99 per person and a $10 access fee for each visit.
How the HFM Chamber Clinic Works
In order to participate, employers must be members of either the Sheboygan County Chamber of Commerce or The Chamber of Manitowoc County. Employers enroll for free, by completing this form for 2018, or this form for 2019. A link will then be sent for employee and dependent enrollments. Each enrollment has an annual fee of $99 that is billed to the employer.
Enrollees will then receive a benefit card in the mail. Enrollees who need to use the HFM Chamber Clinic then call to schedule an appointment and provide the $10 access fee at the time of service.

Why is Holy Family Memorial offering these clinics?
Offering these clinics is in alignment with the HFM vision of promoting a balanced healthy lifestyle for HFM patients encompassing the whole person: mental, social and physical. Having improved access and convenience is a win-win as well for Chamber members.
Who is eligible?
All members of The Chamber of Manitowoc County and Sheboygan County Chamber of Commerce,, their employees and dependents are eligible for the HFM Chamber Clinic. Non-employee members of charitable organizations are not eligible for participation in the HFM Chamber Clinic, as well as any Medicare and Medicaid eligible individuals.
Is there a fee for this service?
The annual investment is only $99 per person. This will be invoiced to The Chamber of Manitowoc County or Sheboygan County Chamber of Commerce member based on the total number of individuals registering. For example, if 10 employees and/or their dependents from a member register, the member (not the individuals) will be invoiced for all 10 individuals. Coverage begins on the first of the month after individual registration is received.
Patients will also have a $10 access fee for each visit which must be paid at the time of the visit. If other services, labs or prescriptions are ordered outside of the services provided by the HFM Chamber Clinic, an employee’s health insurance coverage will apply (and this will be explained by the HFM Chamber Clinic staff).
