Concert Health


At Froedtert Holy Family Memorial our primary care and pediatric doctors and clinicians are committed to helping you improve your behavioral and mental health as well as your physical health. When needed, your primary care and pediatric doctors and clinicians will refer you to our partners at Concert Health for behavioral health care in a virtual setting. Concert Health clinicians work together with your primary care doctor or clinician as your care team to help you overcome challenges and restore a healthy emotional balance. Enrollment with Concert Health will occur within 24-48 hours after your initial referral.

Who will I talk with?

After a referral to Concert Health you are assigned a Collaborative Care Clinician (CCC), a trained, masters-level clinical provider. Your CCC will work closely with you and your primary doctor or clinician to help you begin to feel better. You will have a continuous relationship with your CCC as long as you are enrolled in the program. Your CCC will schedule frequent check-ins to help you set goals, develop and nurture coping strategies and provide support around any prescribed behavioral health medications. They will meet with you by phone or video to help track your treatment progress.

What is a Psychiatric Consultant?

A Psychiatric Consultant is an expert resource available to both your doctor or clinician and your CCC. Your CCC meets regularly with a Psychiatric Consultant to talk about your progress and develop additional treatment options when needed. Your CCC will discuss these recommendations with your doctor or clinician, keeping you informed every step of the way.
