The SSRCH team will work with you and your family to create a plan of care tailored to your needs and wishes. Because hospice care isn’t just for the patient – family and loved ones come along on the journey, and all of you need emotional support and compassionate care.
We utilize a team approach that includes:
- a full-time medical director that is board certified in hospice and palliative care
- registered nurse case managers
- hospice aides
- social workers
- bereavement and spiritual coordinators
SSRCH has served the people of Calumet, Fond du Lac, Manitowoc, Ozaukee, Sheboygan and Washington Counties since 2007. SSRCH is an independent, nonprofit hospice and palliative care provider committed to care that focuses on the whole-person: physical, emotional and spiritual needs. SSRCH has an Inpatient Center in Sheboygan Falls and an office in Manitowoc.
