Paperwork for Pre-employment Health Screen
The two forms for Employee Health Services and the ID Badge form will need to be read through, completed, and brought with you for your pre-employment physical and screen as scheduled with the HR staff. You’ll report to the Transcend, at 1650 South 41st Street, Manitowoc, WI, 54220 (the building to the North of HFM Harbor Town Campus). Our HR staff will also take your photo for your ID Badge while you are here.
**If you are a minor, under age 18, your parents will need to complete the minor consent form.
Paperwork for Orientation
The rest of the forms you’ll need to complete and bring with you to Orientation – many of the forms are “fillable” and you may type in your information prior to printing them.
**You will also need to bring 2 forms of ID, following the guidelines on page 3 of the “Federal I-9 Form” – most frequently used are Driver’s License and Social Security Card; passport counts for both.

Report at 8:20 a.m. to the Skywalk entrance at Western Avenue. You will be in Orientation until approximately 4:30 p.m. You will need to enter through the gated parking lot on 21st St (see Parking Map above). This YouTube video, http://youtu.be/UUcdY5UYVug, will provide you with detailed information about using the secured access to the lot & building, although both will be open the morning of Orientation.
